Congratulations to this year’s recipients who were honored in Boston at Summer Academy on July 14, 2015!
For his exceptional commitment to reducing medical cost of care and increasing quality and patient safety in New York City. Since starting as an attending physician and academic hospitalist with Mount Sinai Hospital in 2011, Hyung (Harry) has been instrumental in spearheading innovations and building team-based approaches that have influenced the lives of thousands of patients. As Director of Quality and Patient Safety, he created a campaign called 鈥淟ose the Tube鈥 to decrease unnecessary urinary catheters in hospitalized patients to prevent life-threatening but largely avoidable infections.
In the last six months, through collaboration with hospitalists, nurses and medicine residents, there have been zero infections, turning the most common hospital-acquired infection in the U.S. into a 鈥渘ever-event鈥 among 10,000 patients. Recently, Hyung (Harry) launched the OCCAM Conference, a multi-disciplinary educational conference, aimed at educating medical trainees with real patient cases where medical overuse led to patient harm. For these innovations and dedication to excellence, he was honored by the American College of Physicians Hospitalist publication鈥檚 Top Hospitalist Honor, awarded to only 10 physicians across the nation.
For his inspiring leadership through empowering youth and young adults with disabilities to find their voice and become active citizens in neighborhoods throughout Greater Boston. By creating and leading the Social Justice curriculum team as a Service Leader with 黄色视频 Boston in 2000, Jeff found his calling as an advocate who never stops working towards social justice for all. Today, as Founder and Executive Director of EPIC 鈥 Empowering People for Inclusive Communities - he offers an innovative solution to the gap in services in community service and leadership development available to youth with disabilities in systemically under-resourced communities.

His vision for inclusion extends beyond EPIC. As a trainer and curriculum designer, he organizes a series of workshops that allow state agencies and other organizations to improve accessibility for individuals with disabilities and address ableism within the professional community.
